Aesthetic Procedures
Face:
Mini midface lift: to avoid large scars and delayed healing time, we use the mini midface lift where part of the incison is hidden in the hairline,and the other in the ear region.The mid face is also lifted from inside the mouth by a hidden incision and dissection over the bone(subperiosteal) and the SMAS or sub muscular aponeurotic layer is tightened;as a result the patient looks much younger for many years.Patient satisfaction is very high with this method.
Micro mini facelift: for the patient who wants only a mini lift for the crows feet and part of the nasolabial region this is a good method;combined with the subperiosteal lift,it gives very good long term results.The scars are very small and hidden along the hairline;it can also be combined with the brow thread lift.
Maxi facelift :for the older patient,this is done for the neck and face using a big coronal(scalp) incision and although the long term results are good there is a big scar in the hairline and the healing time is longer;some much older patients prefer this major procedure to look many years younger;however, there has to be a reasonable balance with the rest of the body and fat grafting of the hands may be needed to match the face.
Non surgical lift: this is done using the laser;in our Sana'a unit we use the carbon dioxide in combination with the mini lift;the areas treated are the perioral region where there are many wrinkles.The carbon dioxide is the best laser for this region and the results are very good, long term.The nasolabial region can be treated with fillers(SMAS,fascia) or botox.There is no perfect or best method to treat the nasolabial folds.All treatments are comparable in terms of long term results.
Thread lifts: We use brow lifting non absorbable threads through hairline stab incisions to lift the brows;the results are good and patient are generally satisfied.The newer Endotine absorbable thread lift for the face is gaining popularity in some countries;however the cost is still very high and may be as much as one thousand Euros for the threads alone;there is no significant advantage using this over our standardized methods which are cheaper and equally effective.
Eyelids and eyebags: The upper eyelids are rejuvenated under local anesthesia using a simple excision of some skin and redraping fat and plicating the septum;the accuracy of our method is well established because we always measure the incision distances pre op and do not make random incisions;our procedure prevents asymmetry and has also been used to correct previous surgeries from elsewhere where the eyelids are asymmetrical after surgery because of poor adjustment and random incisions.The eyebags are treated with minimal skin incision,redraping fat and tightening the septum.
Ptosis surgery: for drooping eyelids , we plicate the levator muscle in milder cases;in more severe cases a muscle sling operation is done using threads or fascia;a frontalis muscle advancement can also be done quite easily.The results are good .
Oriental eyelids: we perform the double eyelid surgery by making new eyelid folds by fixing a few stitches between the eyelid plate and the levator aponeurosis.Some skin is removed;to make sure the eyelids are equal after surgery,accurate pre op markings are made for every patient from the limbal margins to the lower incision site;this is the xfactor marking which we found very useful to get good results.
Acne Scars: In our Sana'a unit we use a combination of lasers and dermabrasion with maintenence with retin A gel, 0.025% for several months;good for mild to moderate cases;severe scars need longer sessions and more prolonged treatment.
Skin maintenence: Post surgery,the patient comes for monthly follow up and is treated with low power lasers, and Glycolic acid peels and lightening creams for several months.This gives an overall good result and patients are very satisfied.
Collagen: Intradermal collagen is used for fine wrinkles and the results last for about one year.
Nose: Nose reshaping surgery is done by trimming the large cartilages and reducing the nasal bones by special rasps;the results are good and an overall improvement in appearance is seen.
The bulky nose can be defatted ;the "potato nose" or rhinophyma is treated with lasers and also nasal shaving;the results are good after surgery.
Crooked nose : treated with nasal osteotomy using 2-3mm chisels, and the nose is straightened;major septal deviations are done by the ENT specialist.
We use the silicon Tokyo nasal implants for nasal augmentation surgery in our Arab and Asian patients and use cartilage as a cover;the implant is always placed under the periosteum of the nasal bone to prevent any movement post op;not recommended for Caucasians who have thin skin.For this group,it is better to use fascia or cartilage.
Cosmetic ear surgery: Otoplasty is done using an anterior cartilage scoring method and also a small anterior incision to reshape and push back the ear;we sometimes use the mastoid suture method for very prominent ears;this is combined with anterior scoring and give good results post op.Other minor ear cosmetic surgery procedures are also done.
Lipocontouring and tummy tuck procedures:The tummy tuck is a relatively simple surgery using a pubic hairline incision to reduce and tighten the waist. It takes about a week for recovery and the patient is advised not to carry heavy weights for at least 12 weeks. Useful for those who need to reduce their saggy lower abdomen after childbirth or other causes.
Lipoplasty for the tummy,breasts,chin,arms,thighs is done using different cannulae sizes using the tumescent method where the tissues are first distendd with saline which contains a small amount of local anesthesia and adrenaline;the procedure is relatively simple and takes a few hours;the healing time is about 5-7 days.We use smaller cannulae to prevent too much brusing or blood loss;the newer method of Vaser liposelection is less traumatic but is also found only in a few hi tech hospitals and is very expensive for most patients.
Male breast reduction by liposuction: simple procedure and good results
also useful for some female breasts where patients dont want a scar;some US plastic surgeons use this method only, with good results
Breast lift: using fascial suspension or a simple periareolar method to lift the sagging breasts
Breast reduction: using a safe and minimal scar method with the periareolar circumglandular plication method;good results for moderate ptosis and may be combined with liposuction to reduce volume
Cohesive gel anatomical implants: using an inframammary incision and placing the implant under the muscle;good results long term and also attractive aesthetically.Less complications like displacement or capsule formation.
Other minor cosmetic procedures: arm lift,fat transfer for defects or augmentation of areas,sweat gland removal or botox for armpits, botox uses including masseter reduction, nymphectomy.
